The witness in AlwaysOn can be a file share and you dont need a separate node running SQL Server hence 3 and 2 are the correct answers

Mirroring: High-safety mode with automatic failover requires a third server instance, known as a witness. If the mirror is in manual failover mode, only two servers is required.

What about the server which is the witness? Shouldn't they both require 3 servers?

There is no withnes in AlwaysAvailability group, 2 is enough
