I would have said A because in chapter 14.1 the very first sentence is this: "The purpose of the starting up a project process is to ensure that the prerequisites for initiating a project* are in place [...]". So *this is an actual stage, therefor I would say A is correct.

As per Figure 14.1 Overview of starting up a project (Prince2 guide) appoint the executive and project manager is the first step (which are both part of the Project Management team). D seems to be correct here

Sure, but if you look a second time, you'll see that they are only one part of the entire process called "Starting up a project", you see the last output of this process is "Plan the initiation stage", so after this last one so-called "Starting up a project" process is set, the next stage will be started
