The answer is A - it should be S42.201B because it is an open fracture.

C- is correct because according to cpt guidelines open fracture or closed fracture is not documented, should coded default closed fracture

The documentation doesn't state its an open or compund fracture, the surgery was an open procedure but that doesn't factor in to the diagnosis of the fracture being an open fracture.
