Looking at the other answers, seems everyone has forgotten the idea of agile is to be agile. call it what you want (hybrid or agile), this project uses change requests and a backlog. B. is th answer by the sheer fact it is leading practice plus the process of elimination with the other answers, if still not sure: A is out (risk register has nothing to do with managing CR's. C. is out because that is not a leading practice CR management process in any approach D. doesn't even make any sense when it comes to directly managing a change request decision/tracking.

'execution phase' - sounds like they are following a predictive lifecycle. Therefore, the PM needs to review the change management plan which is part of the project management plan

I go for A. From the given question we can't understand that it's an Agile environment and moreover, the is no change requests in Agile so B can not be correct. I'd say that answer A is most suitable although it's not a full answer

Anyone can add items to the backlog. Its just that the ordering and prioritization is done by product owner.
