"IP Addressing Mode" is the name of the parameter in CUC, under System Settings -> General Configuration. If you go to the Help -> This Page menu, it says this about that parameter:

IPv4 and IPv6
Default Setting: IPv4"
Per the above, it DOESN'T listen for incoming traffic for IPv6 by default, but the question says "if the Cisco Unity Connection is configured to listen for incoming IPv4 and IPv6 traffic", so to me, that implies that it has ALREADY correctly been set to "IPv4 and IPv6". If it has already correctly been set, then no additional setup is required, so A. There are no IPv4/IPv6 settings in the port or port group settings, so B and C are not a thing, and D refers to a CUCM config, not Unity.
As per Design guide, I believe that A is the correct answer as Unity with SIP integration work either with IPv4 or IPv6 so here if IPv4 configured so unity would work automatically or if IPv6 configured so unity would work automatically also, so setup is not required.
Per the documentation, the key is on IPv4 and IPv6 VS IPv4 or IPv6. Because cisco said "or" A should be the answer as IPv4 is the default

Another tricky question, in the "Server" section of "Port Group" there is "IPv6 Addersing Mode" selection which is for IPv6 you select to use IPv4 or IPv6 for signaling and media, hence the question said you configured the unity, then the best answer to fit will be A.

I think the penny has just dropped - Its taken me half an hour to work this out. Fist the question reads "Regarding SIP intergrations......not SIP integration, so the question implies multiple SIP telephony integrations to the same Unity Cnx cluster, for which Unity is configured to listen to both IPv4 and IPv6, ie the voicemail IP Addressing Mode option on the System Settings > General Configuration page is set to 'IPv4 and IPv6'. In order to integrate with multiple CUCMs you have to configured multiple Phone Systems each with their own Port Groups. One Port Group could be using IPv6 as its preference for Signalling and Media and the second telephony integration/Port Group could be using IPv4 for media and signalling. That would explain the phrasing of Answer B and the use of the word 'and'.
B is correct.

If Unity Connection uses IPv6 or dual-mode
(IPv4 and IPv6) to communicate with Cisco
Unified CM, do the following subtasks:
1. Enable IPv6 on the Unity Connection server.
2. In Cisco Unity Connection Administration,
on the System Settings > General
Configuration page, select an option for IP
Addressing Mode to control where Unity
Connection listens for incoming traffic. You
can select IPv4, IPv6, or IPv4 and IPv6. The
setting defaults to IPv4.
The default settings are IPv4, CUCM will communicate to Unity. Answer A still applies since configurations are not really needed unless you want to use IPv4 and IPv6 in tandem but this is not a port group setting and that disqualify answer B and C.
