If the address is in the table, the frame is forwarded out the port associated with the MAC address in the table. When the DESTINATION MAC address is not found in the MAC address table, the switch forwards the frame out of all ports (flooding) except for the ingress port of the frame

D is correct. when the Source MAC Adress is not found in the MAC Table, the Switch assigns the MAC Address automatically in the MAC Table and forwards the packet normally. when the Destination MAC address is not found in the MAC Table, the switch forwards the frame out of all ports (flooding) except for the ingress port of the frame.

When the destination MAC address is not in the MAC/CAM table, the switch doesn't know which interface it is associated with. Therefore, it floods it out of all frames but the one it received it on (the ingress port).
