Why not D? What meeting minutes are you going to provide them with? The entire projects worth of meeting minutes? Since the last update? I can tell you for one that I would not want to sift through your meeting minutes looking for tidbits of update information here and there. Thats the PM job to put that all together and provide to me. Keep it to what I need to know right now to get me up today on the status.
A and D are correct.
Status report and meeting agenda will help him know about project progress. Status report is a report to stakeholders on the status of the project deliverables, schedule, risks, issues, and more. Meeting minutes describe what was discussed and the decisions made at the meeting.
A and E make more sense, since they have information on the status (A) and which Stakeholders are informed (E) about the project.
It is true that RACI is generally used to provide clarity on the roles and responsibilities assigned to each project team member, but these members are not necessarily informed.
The reason why I go for A and E.

A is used to inform the stakeholders, and E is used to know who "all" stakeholders are and who it should be communicated too and how.
I think the answer provided would indicate what you'd need if you were going to inform a single stakeholder as the question seems to indicate in the first statement. However the question asks which documents should the project manager consult to "ensure all stakeholders are notified of project changes". This changes the context to stakeholders in general. With that being said the correct answers would be C and E. The RACI Matrix and the Communication Plan. Remember, RACI stands for Respsonsible, Accountable, Consulted and Informed. The Communication Plan would specify who should be informed as well.
RACI says nothing about stakeholders, status report does
Project status reporting is a regular, formalized report on project progress against the project plan. ... It keeps project stakeholders informed of critical aspects of project health such as schedule, issues, scope, resources, cost, etc and allows management to take action to address project issues and risks
but stakeholders covers almost anyone involved with the project, if the stakeholder in question is a team member, than the RACI will tell if he needs to be informed on a specific item or not.
