The question states there isn't internet access, which usually does not refer to cellular signals. If a hotspot cannot be set up for what ever reason, tethering allows you to essentially do the same thing as a hotspot, but wired.
"Tethering is the act of connecting two devices together so that one device can use the data of the other. For example, if you have a tablet that can't connect to wireless internet, you could tether it to your cell phone and use that phone's data plan connection to access the web from your tablet."
You cannot have access to VPN without first getting access to the internet. Tethering is similar to hotspot, except this time you're using a physical cable.
