The devil is in the details, the question is essentially asking, why would it be possible for an individual to use a card reader, that would be a access control vestibule or mantrap. An individual would typically utilize a card reader if you want to access a particular room via a mantrap.

This is very clearly B, a vestibule throttles a group so one can enter at a time, in this case requiring a card reader swipe between each person's access. It is not D. Cameras documenting who is entering would require analytics, which are fallible, and while you may have footage recorded for later review, this question specifically says the record is created by a card reader.

Also a smart locker is where you could store your cell phone or other personal items in a secure facility that doesn't allow external electronic devices

Card readers record who swipes their badge using an access control system. Each badge is assigned to an individual in said system. An access control vestibule would enforce each person having to swipe their card. Camera would not enforce badge swiping it just passively records

While this question should be worded a little differently Access Control vestibules would be the best answer: As it relates to security, vestibules are often paired with a check-in window and used to ensure individuals have the proper credentials or identification before they pass on to the next area.

Smart Locker can't be the answer. A smart locker is a cabinet that supports unlocking via a smart card/badge or biometric. The question is asking about staff members enter a facility not a locker or cabinet.
