I chose A since, I was thinking that any prior issues with this team member, would have already been in the Issues Log, thus using that as the said "evidence"

Answer is correct. CompTIA Official guide has an example that aligns with this one pretty well. Page 312. PM assigns a task and issues arise so he goes to the issue log to look at historical entries for this task. "From the earliest days of the project, David instituted the practice of using an issues log to track all matters in question or dispute in the project. "

Doesn't make sense when the issue can be there across multiple sprints. You can't pinpoint with that.
