It could be all three. Transceiver definitely. Wrong polarity absolutely. But a duplex issue also could be a cause. I hope halogen will not be an issue. I will go for transceiver and polarity

If I'm not mistaken, duplexing issues won't prevent connections but will impact performance of the network. You're correct about halogen not being an issue as those only interfere with copper connections and not fiber.
