If there is neither the Session Helper nor the ALG available, how should FortiGate process changing IPs/ports in a SIP session?
B: Firewall policies and voip profiles do not require the ALG in the first step, so it should not be mandatory.
C: By default, voip tries to use the ALG. Session Helper is only used as fallback, if there are reasons why ALG cannot be used. (but I have to admit that this is the main point for troubleshooting SIP)
D: Rate tracking is one of the features that can also be used in flow mode. But all features are available in proxy mode.
