A. A weakness identified within an organization's information systems is the best answer. An analogy is that the oil tank gauge is at LOW, so this is an indication of fuel depletion and dead in the middle of the highway if the driver does not look for a gas station.
I'd say D. I couldn't find a definition of "emerging incident" anywhere, so I assume it is something that you are anticipating can happen to you (your business, if you are infosec manager). Maybe I'm wrong, feel free to correct me. :)
