It needs just 1 condition set with 2 rules in it: Country= US AND City= Redmond, SF

Sorry, my mistake, I took it as if the cities are in an option set. If it is in text then you need 2 condition sets, because of the mixture of AND, OR

B is correct. cannot mix AND and OR condition in the same condition set

B it's correct. Because the second condition evaluate two possible values of the same field. If you don not have practise making business rules this questions could seems tricky.
