I was so tempted to go with B since User 1 appears to already have access given their group membership but the simply fact is that any of these users can request access....even if they belong to a group that has already given them access. User2 does not automatically have access to the app if they an approver for it...just means they been assigned to make decisions. User 3 can request access as well.

User2 is Approver, means already has access to app, Then why need request access for user2?
