You only need one migrate appliance, since it can connect to each Hyper-V host. So it should be 1 and 12

Yes, as below link one appliance can upto 300 hosts https://docs.microsoft.com/en-us/azure/migrate/migrate-appliance And MARS agent needs to installed on all cluster nodes(Hyper-V Hosts) to replicate servers

Yes, It depends on the number of hosts and not clusters. A single appliance can discover and assess up to 300 hosts or 5,000 VMs across multiple clusters.

in the question they mention that the 3 clusters are within 1 domain, therefore they are connected to each other. 1 appliance may do the trick.

wouldn't it be one per cluster - i wouldnt assume that hosts in one cluster can talk to hosts in another cluster, just because they are part of the same domain - they would only need to be able to talk to the DCs in the domain, but not necessarily hosts in the same domain.
