Just login and try to create an On-prem app, then go to advanced settings. For the first question, I believe that due to the app being published through app proxy with the msappproxy.net domain, you would select 'translate URLs in application body' - I just hovered over each option and this one made the most sense to me (but still unsure) For second question, from hovering over 'use http-only cookie', this states it protects against cross scripting XXS. This one was obvious once I saw this.
