from the Link given by C6H6 Now that your client app is configured and you have your application ID, it's time to create a key vault and configure its access policy so you and your application can access the vault's secrets (the Always Encrypted keys). So I agree, that is should be box 1: Always Encrypted by Using Azure Key Vault box 2: reate an Access Policy in azure Key vault

Transparent Data Encryption works on the database level and not on the column level. Therefore the first answer has to be: "Always Encrypted by Using Azure Key Vault" https://docs.microsoft.com/en-us/azure/azure-sql/database/transparent-data-encryption-tde-overview?tabs=azure-portal
