B. Perform a root cause analysis (RCA) with the team members. When issues are detected, and in order to be able to implement corrective actions, you need to perform a root cause analysis to determine the underlying reason behind certain defects, variances, or risks. This method allows you to go beyond showing problematic symptoms to figure out their cause, making it easier to implement the appropriate solution.

From the question, the PM already found out the issue was lack of understanding of the requirements. Why perform a root cause analysis again?
