Iteration planning (Sprint planning) occurs at the beginning of an iteration to define and commit the work to be completed in the upcoming iteration => Team decided to commit more than 10% capacity in the next iteration = Iteration planning. Iteration review (Sprint review) occurs at the end of an iteration to showcase the product increment and to gather feedback on the delivered work => Product owner receiving feedback to add new requirements (not necessarily to be performed in the next iteration) = Iteration review.

you are mixing up SPRINT DEMO (Iteration review) with Sprint Retrospective. SPRINT RETRO is when people talk about lessons learnt and how they will improve in future. Team identified why there velocity went down by 40% and what was the cause. This was a lesson learnt hence discussed in RETRO.

No they don't. Iteration planning should include new requirements.

No. new requirements would come in any time. iteration planning is when the team choose to pick up what to work on this iteration. doesn't make sense that the team decides to give 10% during the review, but it make sense when the stakeholder sees the review and requested to add more features

A team member explains that they will not finish their assignment because they have no experience in the activity → Daily standup The product owner receives new requirements from the stakeholders and includes them in the product backlog → Iteration review The team has decided to commit 10% more than the team's capacity in the next iteration and presents it to the product owner → Iteration planning The team identifies that its velocity went down about 40% in the last iteration and then documents the root cause → Retrospective
