will go with AD. because you dont need to register a risk so quickly. so will go with issue log and change log A. Issue log: The issue log is used to track and manage project issues, including any problems or errors encountered during the project. Updating the issue log with the information about the three invoices with errors will help ensure that the issues are properly recorded and can be addressed and resolved by the appropriate stakeholders. D. Change log: The change log is used to track and document changes made during the project. In this case, the errors in the invoices may require corrective actions or changes to be made. I think it's AE. This is after the fact and not pre-issue. It is an issue and has to be logged. The issue also has to be entered into a backlog register as a prioritized deliverable that needs to be implemented. A risk register is generated before risks occurs, so it cannot be B. A change log is ideally a list of all changes made and tracked in the execution of a project.

Risk is the assumption that a risk may occur. No danger has occurred. The danger has already arisen in the matter. That's why this is an issue. This is why it is not B.

I think it should be AB. You update the risk Register by removing the realized risk into the issue log.

D would have been true but the question says which docs need to be updated as soon as possible. Change log would be updated after the change request has been raised. Risk log would be updated and the remediation plan in it would be to create a change request and then it goes into change log. So, my answer is B instead of D

Change log as after issue log, one need to fix it and that will require update in change log. Risk register is not updated for issue already occurred.
