The problem the PM had was the documentation wasn't updated regularly . You can still store the documents in the PMIS and not update them frequently. I chose A:

Correct me if i am mistaken of the english in the question. "latest version was one month ago", but "project manager knew it was recently updated" Doesnt this imply that the project schedule was recently updated but PM is referencing the wrong version? Wouldnt this then mean that proper version control is needed to ensure artifacts are updated and latest version is acquired.

be careful with the word all. I work with google and we don't share some documents with project coordinators. These stakeholders could change the documents, delete it, alter, tamper, and etc. D is correct.

the reasons others give for picking D. are just odd, to put it mildly. Answer to A may be misleading BUT not seeing that D. does not address the question is bad reading comprehension. PMIS approach does not guarantee having the PIMS version updated regularly.
