It should be A, even moreso because it asks what should you do FIRST. You typically never hire a consultant as the first option, you almost always meet/investigate/etc.

C should be the answer, A cant be, the word 'root cause' is misleading, it asks to perform RCA which is definitely good but then it says to make note of the issue and continue deployment, just making note of the issue doesnt help, employees losing their data is a big issue, action needs to be taken to correct it not just noting it

GPT is saying answer is C but I think D makes sense. The team cannot focus on deployment without knowing the cause and keep loosing the data. So it is a good practice to let the consultant review and share the findings with the team so that they can mitigate the issue.

Answer C is not specific, but Answer A is more accurate in term of find the root cause. I think in answer A, with regard "continue to focus on the deployment" is about corrective action.

If we look at the question again, it stated that the was a missed process which resulted in the lost of data therefore, there is no need to conduct a root cause since we already know its due to a missed process.

you have already miss a process then you still continue to deploy software??? Why you still need a root of cause analysis? you have already know the root of cause is missing a process. The answer should be C.

why not C? becuase it suggests to meet with team and receive feedback as lessons learnt. is it bacuse option A also mention about continue the deployment?
