I think this is C because the quality specifications and deliverables have already been formally accepted, so I don't think it can be B. You wouldn't do an inspection after your deliverables have been accepted, unless the customer as a whole comes back to you and has an issue. It is not uncommon to find a stakeholder complaining about contracted work.

What would the management do here? Shouldn't the PM do some analysis and see where it went wrong if it had gone wrong?

Maybe I am reading the question wrong, but only " The quality specifications for the project deliverables are formally accepted." Nothing says that the actual deliverables were formally accepted. I went with B.
