B. it is too late to update the risk management plan as the project has run into an issue. when a risk becomes an issue, the first step should be to work out possible solutions.

D is absolutely wrong since it is no longer be a risk but an issue, so you cannot log it into risk management plan (also you can log risk into risk register, not risk management plan), so B

As a project manager your first response to any issue is to find a work around. Escalation is only a secondary response so I go with B

Its was Risk hence there should be a response plan already, which must have discussed with stakeholders for response , but seem it did not work and turn in to issue . So this needs to be escalated to next level I go with A
