Selected Answer: C
To improve the stakeholder relationship, especially when there is an indication that the stakeholder feels left out, it is essential to engage and communicate with the stakeholder directly. C. Engage the stakeholder to solicit more information before responding to the request. By directly engaging with the stakeholder, the project manager can better understand the stakeholder's concerns, needs, and expectations. This will provide an opportunity for the stakeholder to feel heard and valued, fostering a better relationship and improving collaboration. Once the project manager has a clear understanding of the request, they can then decide on the appropriate next steps, whether it be to update documents, initiate changes, or take other actions.

The PM should gather information first. From the question, it looks like the stakeholder is already identified.

I think we need to engage the stakeholder first to know why he was omitted from the stakeholder list to find if truly he /she is met to be in the list, after that we update the stakeholder list. But first we must ENGAGE....

adding a stakeholder to a document does not improve a relationship. that is the key ask in the question. Maybe if question asked what should you do first as the question may have made A. correct.
