C. is incorrect. It did not say designer did NOT document. It said he did not provide it to the team. One could still document and fail to provide the document.if yo picked C. he could still not provide thanks to a broken process. therefore agile leader should review the process to figure out next steps. which may require updating the process or training.

in Agile, individual and interaction over tools and process. I select C, asked the team to document the design. there is documentation in Agile but not comprehensive, the documentation is bare minimum.

is "C. Ask the team to document the design" wrong because Agile doesn't prefer documentation? IRL There has to some level of doc. used even in Agile. -.-

exactly... There is a difference between real life and what the exam expects. In real life, the first thing we do is to ask for some documentation, not like a manual but at least something that can be used for future members.
