After setting up a connection to an external ALM system, no initial synchronization of existing artifacts is performed. This should prevent the creation of large amounts of data in Test Manager without any prediction of how long it will take. However, every time an object is updated in the external tool, it will be created in Test Manager if it doesn't exist.
